Office of the Registrar Forms
***While access to campus is limited all completed forms should be emailed as scanned copies to the Student Records Office at registrar@westmont.edu from your westmont.edu email account. We cannot make changes to your academic record from forms sent from your personal email. If you do not have the ability to send a scanned copy of the completed form you may instead send a picture of the form as an attachment as long as the picture is clear and the handwriting is legible.
***Once received, the Student Records Office staff will work to secure the necessary faculty signatures and will update you when the form has been processed.
